I voluntarily and freely give my consent to the Cebu City Police Office and its Data Protection Office (DPO), Personal Information Controllers (PICs) and Personal Information Processor (PIPs), to collect, store, record, organize, process, update or modify, retrieve, use, consolidate, block, erase or destruct my personal information provided herein.

I Also give my express consent to the Cebu City Police Office to verify and validate the information I have submitted in the connection with my application for a Police Clearance.

I understand that the data I have submitted shall only be used for the processing and issuance of a Police Clearance.

I am fully aware that my data and biometrics will be cross-checked against the PNP warrant database as part of the vetting or screening process. I am aware that any crime or violation that I commit now and in the future shall form part of my personal record/information.

I hereby authorize the recipient of my Police Clearance and to verify its authenticity online.

The Cebu City Police Office and its Data Protection Officer (DPO), Personal Information Controllers (PICs) and Personal Information Processor (PIPs) shall not disclose my personal/sensitive information to any party without my express consent. I hereby affirm my rights under the Data Privacy Act including the right to object to processing of my data, the right to access my data, the right to correct any inaccurate data and the right to erasure and blocking of data.
